- Comprehensive X-rays (every 3 years)
- 20% off oral surgery, fillings, sealants
- 15% OFF: Crowns, Root Canal Therapy, and other Major Dental Treatments
Annual Membership Fees:
- 1 Member: $369
- 2 Members: $355/kid
- 3 Members: $335/kid
- 4 Members: $320/kid

- Membership fees must be paid in full before benefits or discounts apply
- Co-payments are due at the time of service, and some cases may require a deposit
